Healthcare systems and the way safe, quality health care is delivered are continually changing to better serve patients and communities. Professional nursing practice is a large component in the healthcare system today. Back in the 1960s, professional nursing leaders tried to adopt the bachelor degree programs as the only educational track to become a registered nurse (Creasia & Friberg, 2011). Due to nursing shortages and demands this motive did not hold fast. Individuals entering the nursing profession today must first decide which educational pathway to take to become a Registered Nurse (RN).

There is a shortage of all health care professions throughout the United States. One shortage in particular that society should be very concerned about is the shortage of Registered Nurses. Registered Nurses make up the single largest healthcare profession in the United States. A registered nurse is a vital healthcare professional that has earned a two or four year degree and has the upper-most responsibility in providing direct patient care and staff management in a hospital or other treatment facilities (Registered Nurse (RN) Degree and Career Overview., 2009). Transitioning from nursing school to working in a hospital setting can be a challenging time for a new graduate. Due to the nursing shortage, new graduate nurses are being hired with little to no experience. This is overwhelming for new nurses, especially when they are not getting adequate support or training from the hospital. The amount of stress, pressure, and lack of training is leading to a high turnover rate for new graduate nurses. With patient acuity on the rise, new graduate nurses that are filling these vacancies in the hospitals, need to be competent nurses to provide proper and safe care to the patients.

According to Creasia (2010), “The first baccalaureate nursing program was established in the United States at the University of Minnesota in 1909” and many programs were 5 years long (p. 4). Today, the BSN program is a 4-year college degree offered at universities, private schools and community colleges throughout our nation. The BSN degree graduate has many advantages over an ADN degree graduate and these might include: career paths that are only open to nurses with a BSN degree including administrative positions and minimum requirement for advanced practice nursing (APN). Furthermore, the BSN graduates get extensive training in components that might include: quality and patient safety, evidence-based practice, information management, clinical prevention/population health, and professional values, all of which are essential as nursing care becomes more complex (Creasia, 2010, p. 4). One of the biggest disadvantages of having a BSN degree is that the legal scope of practice is undifferentiated due to being awarded the same license as an ADN graduate “hindering the reward system for leadership responsibilities” (Creasia, 2010, p. 16).

Bachelor degree nursing programs are highly regarded for the skilled, clinical thinking, and professionalism of nurses they provide for the community. Obtaining a baccalaureate in nursing provides the same skills and knowledge encompassed in most associate’s programs, while allowing a broadened look into research, public health, management, and humanities (AACN IENP, 2012). Many facilities are now providing incentives for nursing staff to continue their education; from tuition reimbursement, an increase in pay, and played time off for the sole purpose of education. Research has proven that with a more educated

ADN graduates are proficient in entry level nursing skills and their skill set could be adequate for their particular setting. If the setting was that of an intensive care unit, emergency department or post anesthesia recovery unit however, the nurse would require advanced critical thinking skills that are developed in baccalaureate programs.

There are only five years left before the 2020 Recommendation to increase the number of registered nurses with Bachelor of Science in Nursing degree from 40% to 80%, would take effect. Nurses, who have no BSN credentials and are not yet enrolled in any educational program, are aware of the employers’ preference for BSN prepared nurses. Cognizant of the financial implication of enrolling in a BSN program, the Institute of Medicine (IOM) recommended that

The need for expansion in their roles for higher levels of education and an improved system of education are required for nurses now for meeting the diverse needs of the patients and to provide better care. Improving the education system and achieving a more educated work force is increasing with nurses having baccalaureate degrees , that is, a traditional Registered Nurse to Bachelor of Nursing program or a traditional 4 year Bachelor of Nursing program or a program that allows seamless transition from Associate in Diploma Nursing to Bachelor of Nursing.

The implications for BSN-prepared nurses in addition to those of RNs with an associate degree

Initially, I have to admit, I was torn between my thoughts of the proposed goal of 80 % of nurses being BSN prepared by 2020 in the healthcare workforce. However, after conducting further research on the topic, it is evident that this goal can only result in positive outcomes. As Edwards (2012) discusses, BSN and MSN prepared nurses are more equipped to manage the care of individuals, families, and healthcare facilities than those who are not. Some may argue that experience is key in the healthcare field, not necessarily the number of years of education, and dismiss the proposal. “Throughout the last decade, policymakers and practice leaders have recognized that education makes a difference” (Impact of Education, n.d.). Hospitals are also trending towards preferred hiring of bachelors prepared nurses for their workforce. Even national organizations are jumping on the bandwagon requiring “all nurse managers and nurse leaders to hold a baccalaureate or graduate degree in nursing by 2013” (Impact of Education, n.d.), likewise the Institute of Medicine has also recommended that all hospitals have at least 80% of their nursing staff with a BSN or higher by the year 2020 (Impact of Education, n.d.).
