Problem/Issue/Challenge Reported by the client The client Amelia Yee-Jones has come to counseling because she is not "feeling like herself," has difficulties getting out of bed, and is concerned that someone might be "out to get
2 Running Head: FINAL PROJECT I DRAFT ONE her." Her spouse Juanita confirms her statement and reports that Amelia stares off into space and is sometimes unable to "snap" out of it. B. Problem/Issue/Challenge affecting the client Based on her intake interview, Amelia presents some behavioral, cognitive, emotional, and psychological issues. The behavioral issues include anger or aggressiveness, decreased energy, dissociative states, and ADL problems. She reports having cognitive impairment, orientation/memory, and disruptive thought patterns. Emotionally, she writes about having anxiety, depression, hopelessness/helplessness, grief, guilt, irritability, paranoia, severe mood swings, tearfulness, and worthlessness. Psychologically, she presents issues related to appetite disturbances, panic attacks, sleep disturbances, and obsessions/compulsions. C. Experience and Demonstration of symptoms. Amelia's symptoms demonstrate her inability to manage and regulate her emotions. She reports struggling with her mood, being nervous, lacking Motivation, and having little or no interest in life. She noted constant tiredness with deficient energy levels. She states that these symptoms are beginning to interfere with her job and life, while her profession and family seem to activate her symptoms. Pressure to deliver a project by a due date, family gatherings, holidays, and being alone at home are some of the identified activators of her stressors. D. Quantitative Assessment 1. WHODAS 2.0 (Disability Assessment Schedule 2.0): self-administered, 36-item version: Amelia scores 3.25 on a scale of 5 indicating that her symptoms moderately impair her ability to function and work effectively.

Level 2 - Anxiety – Adult (Short form): Amelia scored 72.9, indicating a severe level of anxiety after the DSM-5 Self-Rated Level 1 Cross-Cutting Symptom measure predicted either mild or more significant anxiety. However, based on the Severity Measure for Generalized Anxiety Disorder – Adult, she scored 3.2, indicating she has severe generalized anxiety disorder but scored moderate for separation and social anxiety disorders. 3. The Personality Inventory for DSM-5 – Brief Form: Amelia's overall score indicates that she has a very mild personality dysfunction. The result conforms with the personality trait domain, where she scored less than two on the average domain on all five personality traits. 4. Severity Measure for Panic Disorder – Adult: Her total score is 3.0 indicating that she has a severe panic disorder. E. The qualitative assessment presents the client's issues as she sees and understands them, while the quantitative assessment uses data to analyze her symptom objectively. Her intake interview indicates that she finds it challenging to maintain a job, supported by the severity of her anxiety disorder. The data from both assessments provide an overall level of clarity to the client's challenges and pave a way to define her goals. F. Considerations 1. WHODAS 2.0 is an appropriate assessment because the client disclosed being diagnosed with dyslexia and struggling with English at school.
