In the past few years there has been increasing discussion about how to provide adequate care for the increasing number of veterasn who are eligible for care through the Veterans’ healthcare administration (VHA). There are concerns is that the VHA is not providing the level of access, efficiency, and quality of care that veterans expect. Lee & Begley, (2016) suggest access to care for the veteran population may be resulting in poor health outcomes. In response to these concerns, the Veterans ' Access to Care through Choice, Accountability, and Transparency Act (VACAA) of 2014, also known as the Veterans Choice Act, was created to improve Veterans’ healthcare. The VACAA proposed to do this by expanding the number of options veterans have for receiving healthcare, by providing access for healthcare at non-VA care centers as well as providing for an increase in staffing at VA facilities (U. S. Department of Veterans Affairs, 2016).

Rates of trauma and mental illness are reported to be disproportionately higher among American veterans, especially those of the recent wars in Iraq and Afghanistan. The barriers to care after civilian reentry further disadvantage this already vulnerable population. The wars in Iraq and Afghanistan have been the longest sustained US military operations since the Vietnam era, sending more than 2.2 million troops into battle and resulting in more than 6,600 deaths and 48,000 injuries. Veterans are at risk mental health challenges, as well as family instability, elevated rates of homelessness, and joblessness. Veterans have disproportionate rates of mental illness, particularly posttraumatic stress disorder (PTSD), substance abuse disorders, depression, anxiety, and military sexual trauma.
The SMVTA Center works with states to bolster and support the behavioral health systems for active duty military service member, reservists, veterans and their families. They act as a liaison between agencies and the service members, specifically federal, state, territorial, tribe, local community, public, and private agencies. This organization monitors trends in behavioral health issues in relation to prevention, treatment and recovery support, and provides consultation, training and technical assistance to these agencies to provide the latest and best treatment
Both articles identify the issue of providing mental health services for veterans with an extra emphasis on those that served in Afghanistan and Iraq (OEF/OIF veterans). It is no surprise that returning veterans suffer from both visible/invisible (physical and mental) wounds. Most veterans have this “high” expectation that they are going to receive quality care from both the DoD and VA. Unfortunately reality steps in where veterans are slapped in the face because they are receiving a lack of poor quality care all while jumping through Beuracractic hoops. The challenges faced to access these services include resistance, stigma, lack of professionalism, and geographic and/or regional disparities in the distribution of services resources and/or benefits, and the system simply refusing to change.
The mean age for the participants in the study was 45. The study also took in account the time of service from the veterans, 60% served in the 1970's, 18% in the 1960's, and 18% in the 1980's (Carlson, Gavert, Macia, Ruzek, & Burling, 2013). The VA has made great stride in providing specialized services to veterans such as Traumatic Brain Injury (TBI), Military Sexual Trauma (MST), and Mental Health treatment. In fact, the VA is leading the field on Post-Traumatic Stress Disorder (PTSD) research, but now that many of our men and women are returning home from war, the commitment that the VA made to provide accessible health services and a smooth transition from military life back to civilian life to these heroes and their dependents are not being granted in a timely manner. In 2003, The Joint Commission made one of their first goals to improve the accuracy of identifying patients to reduce or eliminate patient identification errors. This continues to be an accreditation requirement. Their recommendations to do this are to use at least two patient identifiers when administering medications, and when providing treatments or procedures. Acceptable identifiers may be the individual’s name, an assigned identification number, telephone number, or other person-specific identifier. Patient room number or physical location may not be used as an appropriate identifier. Healthcare provides should re-identify the patient with each encounter, each medication pass, and each procedure. There have been procedures and protocols throughout the country have been put into place to make the care provided to patients safer. Another element of this requirement is that all containers should be labeled in the patients presences after using the patient identifiers
A service member’s emotional and psychological issues are not like malfunctions of a piece of equipment and must not be treated as a troubleshooting problem in a technical manual. And yet this seems to be the only way the military, as a whole, knows how to address the issue of a service members mental health. It requires a holistic approach that is not currently embraced by the military. Historically, the military has been ill-equipped to handle the shifting nature of the psychological issues created by introducing a service member to the battlefield.
The monthly review process has been in place only for about a year so increasing the numbers to meet requirements has not been done before. The Project Management Office has requested a specific time to complete this project and has established a specific requirement, is to get the number of monthly reviews completed above 90%.
