A partnership is a relationship between two or more individuals, groups or organisations that work together of a period of time, it is essential for effective partnership working for each party to work towards the same goal. This approach should highlight shared aims whereby responsibilities are highlighted and everyone is aware of their contributions towards achieving outcomes; in essence this should build and maintain trustworthy relationships where issues can arise and solutions can be found when decisions are made as a partnership rather than independently. We work closely and staff often in twos, as opposed to lone working, as this encourages individuals to promote a positive environment for citizens, family, friends and visitors whom enter the home. Additionally the support staff is a small team so it is essential that we are honest and can address any issues they may have in the workplace in order to progress and move forward with mutual understanding. Furthermore it is important to use each other's strengths as mentioned above as a base for supporting citizens in their home environment; for example one colleague may be better explaining and communicating to particular individuals when they are feeling low, having an episode or relapse, whereas another member of staff may be better motivating the citizens to get out into the community. Clear methods of communication is beneficial to lead successful partnership working, these can involve communication books, email, fax, over the phone, face to face, daily plans, team meetings, rotas and diaries. In addition, I recognise that the support staff, who work with the citizens on a daily basis, are the best sources of advice and information needed when issues arise in relation to the citizens living in the community. Confidentiality is necessary in any Health and Care Social setting because it maintains between the individuals and the organisation. An environment of trust encourages people to be open and honest with those who care for them. They provide all the details necessary so that they receive the best care possible. Staff working in partnership with a service user’s family will become more aware of families' individual needs, and know more about an elderly person's home context, thus enabling them to be aware of how the care home is different or similar to their own home and understand the elderly person’s behaviour with this in mind.
The features of effective partnership working are to ensure there is good communication between all parties that ensure a high standard of care is delivered to the individual involved at all times. It ensures each party is covering all area’s and that all needs are met and that all partys have a good knowledge of who is delivering what to the individual and the contacts they have if other issues may arise and the access they have to these.

Others people may be able to provide useful information to support you in your work and you may be able to provide useful information to support them in being part of the individual’s lives. This is good partnership working. If there are communication difficulties with service users a carer or family member can share information with you about how you can best communicate with an individual.

They will have details about the individuals past history and life which will all be taken into account when establishing the individual’s plan of care and support. An example might be if there are communication difficulties. A carer or family member can share information with me about how I can best communicate with an individual.
