Katrina the first half of 2016 you were under the supervisor of a different supervisor who noted that you continue to maintain familiarity with policies, confidentiality, agency programs like CDS, In Home Services, ADC, RCF and outside resources, as well as systems such as PROD, FRED, Web tool and Case Compass. She also noted that you adhered to all policies and includes HIPPA forms into cases or you would explain why they could not be obtained. It was also noted that you effectively use your experience and knowledge to provide information and counsel to assist the reported adults, but also when working with MMAC, EDL, DSS and other agencies. In addition, you volunteered to have a new worker shadow you in June 2016 as well. Katrina you usually go above and beyond mere shadowing with working with new employees. Katrina you take the time to explain to the new worker all actions you take. You give excellent background information and related policies with a pleasant attitude, thus providing the new employee with excellent and valuable know-how for her job. Katrina you maintained a great working relationship with co-workers, outside entities and your supervisor, whom you kept informed of all pertinent issues. Katrina you always responded to inquiries, as they were received and handled stressful situations calmly. Katrina you immediately accepted and acted on the assignment to make wellness calls to 7 clients on 6/17/16 to check on heat related issues.
During the second half of this rating period you continued to maintained and effective working relationship with your coworkers and team members but sometimes fell short of an effective relationship with customers and other external stakeholders. Katrina you have handled tenses/stressful situations calmly in order to reach resolutions but there were time when you fell short of resolving stressful situations. Katrina as stated above you do act independently without specific construction but during this second half of this rating period you fell short in consulting with APCS or appropriate cases.
Katrina you have complied with Department policies, procedures and programs. You are prepared for conferences and during the first half of this rating period you kept your supervisor informed of your schedule. Katrina you used, DELTA and your calendar to plan your whereabouts and you submitted all required forms and documentation timely. Katrina your cubicle was well organized.
Katrina you continue to struggle with time management this is reflected by not meeting time frames in status reports and not completing investigations within 60 calendar days. Ms. Allen has not submitted expense reports within the required time frames. Katrina you have not attended some mandatory trainings. Katrina you have maintained regular and reliable attendance to perform job functions but sometimes you fell short of completing your job functions.
