Cultural Considerations in Health Care Essay

“Health is influenced by culture and beliefs” (NRS-429V, 2011, p. 1). In order for the nurse to properly care for the patient, she must know and understand the patient’s culture. “Cultural care is a comprehensive model that includes the assessment of a client’s cultural needs, beliefs, and health care practices” (NRS-429V, 2011, p. 1). It is not enough to just know where the patient lives or where he came from. The nurse must embrace the concept of cultural competence and cultural awareness. This requires not only the awareness of the cultural beliefs and values of their patients, but also

“Americans can take come pride in the fact that attaining what the medical profession calls “cultural competency” is a goal of most health care institutions. However, achieving this goal in today’s health care environment, filled with diverse patient and provider populations, is no easy task. American hospitals are increasingly being staffed by and serving diverse populations. This creates the ideal breeding ground for conflict and misunderstanding among the staff and inferior patient care” (Galanti, 2011). To gain a more thorough understanding of this concept, I will be giving four examples or viewpoints that are completely different, when looking at the Hispanic belief against the Native American point of view.

In rural Iowa, there are few individuals who are knowledgeable of the Hispanic culture due to the small number of Hispanic individuals that have sought out care in the past. Recently, there has been an influx the number of Hispanic Americans seeking care in this area, making the delivery of culturally sensitive communication and care an important topic. According to DeNisco & Barker (2016), the nature of nursing care encompasses the need to be aware of cultural diversity (p. 581). Meaning that nurses need to strive for cultural competence to reach the societies expectations in the delivery of nursing care (DeNisco & Barker, 2016). We are going to take a look at how nurses can make a few changes that will make providing culturally competent care to the Hispanic population easier in these settings.

In 1988 Giger and Davidhizar created their Transcultural Assessment Model (TAM) to facilitate “…the discovery of culturally sensitive facts… (and) provide culturally appropriate and competent care.” (Giger & Davidhizar, 2002) The TAM was developed for an undergraduate nursing curriculum to train students to assess and provide care for patients that are culturally diverse. This paper will employ the six cultural phenomena defined by TAM to assist in identifying cultural attributes of Mexican Americans that should be considered by nurses in assessment and care.

Cultural competence in nursing is imperative for effective patient care. A nurse must know his or her own values and beliefs as well as knowing about a patient cultural practices in relation to healthcare. In order to deliver nursing care to different cultures, nurses are expected to understand and provide culturally competent health care to diverse individuals. Culturally competent care is tailored to the specific needs of each client, while incorporating the individual’s beliefs and values (Stanhope & Lancaster, 2006, p. 90). With the large increase multicultural population in the United States, nurses encounter patients with differences in healthcare beliefs, values and customs. To provide adequate nursing care, nurses must be aware of these differences. They must respect and acknowledge the patient’s culture. To do this, nurses need education on cultural competence to ensure patient satisfaction and better patient outcomes.

Statistics show that the Columbus, Ohio metro area is becoming more diverse. Even though the majority of the population is made up of Whites, that percentage is decreasing while the percentage of Blacks, Latinos and Asians is increasing (Diversity Data, 2012). I work in an outpatient unit that serves a diverse patient population, and Leininger’s concepts of transcultural nursing highlight the importance of learning about the cultural influences that affect the health and wellness of ethnic populations (Andrews & Boyle, 2016). Transcultural nursing addresses cultural influences such as values, beliefs, and behavior and examines how they affect health and wellness. In this paper, I will discuss perceptions of health and illness in the Hispanic community, examine the overarching issue presented in a video that I reviewed, discuss what can be done to overcome the issue in the video, and share a barrier that I experience in my nursing practice and how I overcome the barrier.

America is composed of a variety of people who all have different beliefs and traditions, as well as their own unique set of beliefs regarding their healthcare practices. One such group is the Hispanic Americans who have their own very diverse cultural group. As individuals, they are just like anybody else in any other culture. On the other hand, although the healthcare provider is able to speak their patient's language, providers often fail to consider the patient's education level and may at times use medical jargons that the patient may not understand, thus resulting in a failure to communicate with their patient. Healthcare provider may lack the skills and knowledge in understanding their patient's culture other than their own. This often results in the medical provider "pushing" their beliefs onto their patient and if the desired results are not realized, this results in mistrust of the healthcare provider and the healthcare system. To increase cultural competence among healthcare workers and increase the amount of minority's access to healthcare, medical schools and medical facilities are incorporating "Clinical Cultural Competence Interventions." The goal of this program is to provide "educational and training interventions to equip healthcare providers with the knowledge, tools, and skills to better understand and manage socio-cultural issues in the clinical encounter," (Betancourt, Green, Carrillo, and Ananeh-Firempong, 2003, p. 298). In the last twenty years, the rising number of disparities in health and healthcare has increased simultaneously with the influx of minorities within the population (Baldwin, 2003) A4. Nowadays, nurses not only need to know how to care of their patients, but they also must be able to care of patients from other cultures with many beliefs and values. Cultural views of individual influence the patient’s perception and decision of health and health care (Creasia & Parker, 2007). In order to care for people across different languages and cultures, nurses need to develop cultural sensitivity, knowledge, and skills.

Today when people move across continents with the help of technology their culture and heritage moves along with them. Almost each and every continent is populated with people from different nations who have diverse traditions and cultures. Thus knowledge of health traditions and culture plays a vital role in nursing. People from different cultures have a unique view on health and illness. Culture-specific care is a vital skill to the modern nurse, as the United States continues to consist of many immigrants who have become assimilated into one culture. I interviewed three families of different cultures: - Indian (my culture), Hispanic and Chinese. Let us see the differences in health traditions between these cultures.
