“Blount Senior Care Partners uses a team based approach that focuses on continuity, patient independence, and clinical excellence” (Blount Senior Care Partners, 2014). Blount Senior Care Partners provides care in an outpatient geriatric clinic, several assisted living facilities, rehabilitation and long term facilities within the community and during acute inpatient hospital admissions at Blount Memorial Hospital. Currently, the care of inpatient hospitalizations is exclusively managed by the physicians of Blount Senior Care Partners. However, the Adult-Gerontology Acute Care Nurse Practitioner (AG-ACNP) possesses the knowledge and skills to be a valuable team member of Blount Senior Care Partners in both the

Identified market needs and gaps in current services
Before hospitalists were introduced into healthcare, the primary care physician provided care for patients in the office, hospital, and, when necessary, the skilled nursing facility (Blount Senior Care Partners, 2014). “Even though the patient’s clinical circumstances were changing, a caring advocate with full knowledge of the individual 's needs and goals could smoothly transition the care of the patient” (Blount Senior Care Partners, 2014). Much has changed in the healthcare field, leaving behind a fragmented system for the vulnerable geriatric population. Blount Senior Care Partners aims to recreate this level of continuity for the patients and families in Blount County and the surrounding area (Blount Senior Care Partners, 2014).
Description of customers and services provided
“Specialized geriatric medicine helps coordinate care between physicians, therapists, social workers, family members, and community resource agencies to provide the best care possible” (Blount Senior Care Partners, 2014). Blount Senior Care Partners serve seniors of Blount County and surrounding East Tennessee counties and primary care physicians in the community. The challenge America now faces is the number of people reaching retirement will double in number by 2030, and the U.S. population will increase almost 20 percent ("Our Aging Nation," 2015). The goal is for the elderly to maintain and live with independence and dignity, as well as, provide a wide range of professional health and social service expertise, home care, and residential support and services that will be needed ("Our Aging Nation," 2015). Since the number of caregivers needed for this population will not be able to meet the demands other resolutions are considered necessary.
The long-term healthcare system has an infrastructure that is unfriendly and difficulty to navigate. With a growing number of frail older people living in the United States, the pressure to find a more affordable way to care for them will mount. Nearly 12 million people need some form of long-term care (LTC) and half are over age 65 (Wilber, 2014, Class Session 2). As mentioned in the article “Waiting in the Dark with Dad,” a large percentage of American healthcare costs fund medical procedures and medications to keep terminally ill people 65 and older alive (Lopez, 2014, p. 1). Steve Lopez experienced this with his 85-year-old father, Tony, who lived in a nursing home that was a place meant to keep his heart beating, but not actually improve his health and well-being. This is the harsh reality of the long-term care industry that many have faced, including experts in the field such as Dr. Robert Kane, author of It Shouldn’t Be This Way: The Failure of Long-Term Care. Long-term services and supports (LTSS) are supposed to maximize the quality of care and the quality of life for the individual (Kane and West, 2005, p. 169). When that focus is lost, it is the responsibility of healthcare professionals, like myself, to bring that issue back to the forefront of policy making.
According to Statistics Canada, aging population has steadily increased since the mid-1960. The age of 65 and older comprised 15.7% of the country’s population. Promoting health in these aging population is very crucial to minimise the severity of health complications that might occur in the long run. Gerontological Nursing involves the care of aging people and emphasize the promotion of the highest possible quality of life and wellness throughout the lifespan (Eliopoulos, c2014 p73). Aging people need Gerontological Nurses who are knowledgeable and willing to promote health and optimum quality of life with the consideration of their core needs such as physiological balance, connection and gratification (Eliopoulos, c2014 p78).
Initiatives like AAI are very important to means to providing accessible health care for the elderly. No center is within an hour’s distance from an elderly Arkansans’ resident. Teamwork is the key. Physicians, nurses, social workers, and other health professionals all work together as a team for the benefit of their patients. Advanced degreed nurses act as associate directors. Many organizations are needed to cover the amount of patients that need and will need care and prevent of disease control. (2010). “A greater amount of resources from health and community services is necessary as the number of elderly persons in the community increases”. As the elderly population increase so will the complexity of care that these individuals will require. Many of these patients are living with several illnesses that will require a high level of care and expertise of healthcare providers. In this paper I will discuss the challenges this patient populating faces and potential way we can improve the care we provide.
