The American Society of Addiction Medicine (ASAM) assessment is an inclusive evaluation tool that assists clinicians to structure treatment priorities and recommend the least intensive treatment plan possible. The assessment is divided into 6 sections or dimensions that allow the clinician to ask specifically designed questions in a comprehensive biopsychosocial evaluation to assist with client placement and treatment planning. Each section of the assessment corresponds to a dimension in the ASAM criteria guidelines: Dimension 1: Acute intoxication and/or withdrawal potential. Dimension 2: Biomedical conditions and complications. Dimension 3: Emotional, behavioral, and cognitive conditions and complications. Dimension 4: Readiness to change. Dimension 5: Relapse, continued use, or continued problem potential. Dimension 6: Recovery/living environment. (ASAM,2023) ASAM Criteria is used for Patients with Addiction and Co-occurring Conditions that are interfering with their ability to live their life unincumbered. Within each dimension in the ASAM, there are severity or intensity levels that would need to be determined based on the answers to the assessment from the client. Each dimension has a risk level that starts at “0” and goes up to “4”: (ASAM,2023) Risk Level 0 – A chronic problem is stabilized, it is a non-issue, a very low-risk issue. Risk Level 1 – A chronic problem should be stabilized quickly, functioning problem should be quickly restored, mild discomfort or struggle, mild Risk Level 2 – A chronic problem is affecting the client and is showing noticeable impairment in functionality, experiencing moderate risks or difficulties, client is still able to cope with and understand support services, moderate.
Risk Level 3 – A chronic problem with difficulty coping with & understanding issues, serious difficulties and impairment, near imminent danger, substantial. Risk Level 4 – A chronic problem with severe difficulties or impairment, serious signs and symptoms, very poor ability to cope with or tolerate illness, severe. (DDAP, 2022) As an example, for Dimension 1: Acute intoxication and/or withdrawal potential. Risk Level 0 – a client at this level would show no sign of intoxication, would not present with withdrawal symptoms, or is able to cope and tolerate the withdrawal symptoms they are experiencing. Risk Level 1 – a client at this level would show mild to moderate intoxication, symptoms that interfere with daily activities, they are able to tolerate the withdrawal symptoms they are experiencing, a minimal risk. Risk Level 2 – a client at this level would show difficulty tolerating withdrawal symptoms, would be somewhat intoxicated, does not show signs of imminent danger to self or others, moderate risk Risk Level 3 – a client at this level would be very intoxicated, would show severe signs and symptoms of withdrawal, has a poor ability to cope with withdrawal symptoms, imminent danger to themselves or others Risk Level 4 – a client at this level would be extremely intoxicated, would have severe withdrawal symptoms, continues to pose a danger to themselves or others, severe risk (SAMHSA , 2023) By utilizing all of these dimensions and risk levels a clinician can determine the level of care that a client may need in their rehabilitation process.
