Ruth Kerubo Oguku MondayDec 18 at 7:38am Manage Discussion Entry Searching Database Clinical Issue The clinical issue of interest that I would like to research is the use of electronic health records (EHRs) in patients admitted to healthcare facilities versus traditional paper-based medical records and prescriptions. Implementation of EHRs has revolutionized patient care, ensured that patients’ information is accessible, and promoted coordination among various medical professionals. EHR has facilitated effective communication between nurses and patients and offers an avenue for mutual understanding among different medical professionals. Nurses can effectively record and share patient data with other healthcare staff using the same system. The implementation of EHR has ensured more accurate diagnosis and treatment of patients, and thus, medical errors can be avoided, hence improving patient outcomes. PICO(T) Question In patients admitted to a healthcare facility that shifted from paper-based medical records and prescriptions to electronic health records (P), does the use of electronic health records (I) instead of paper-based medical records and prescriptions (C) minimize medical errors and improve patient outcomes (O) within one year of its implementation (T)? Search Results After coming up with my PICO(T) question according to my clinical issue of interest, I commenced my search at PubMed and ScienceDirect databases on the Walden Library using the phrase “electronic health records.” PubMed yielded 69,108, while ScienceDirect produced 8,476 recent nursing and health professions research articles, particularly those published in the last 5 years. Then I included the Boolean operator “or” and the abbreviation EHRs and searched for “electronic health records or EHRs.” Pubmed and ScienceDirect yielded 34,535 and 1,144 articles, respectively. Then, I further added other words while connecting them with the Boolean operator “and,” to be more specific. Thus, I searched for “electronic health records, or EHR, and efficiency.” The PubMed database yielded 2,700 articles, while ScienceDirect produced 1,577 articles. When I searched for “impact of electronic health records (EHR) on healthcare quality and safety" from the PubMed database, I got 12 articles, of which I selected one, authored by Upadhyay and Hu (2022). On the other hand, on the same search, I got 237 articles from the ScienceDirect database and selected three of them authored by Kartika et al. (2021), Harmon et al. (2023), and Devin et al. (2021). Strategies to Increase the Rigor and Effectiveness of the Database Search on the PICO(T) Question The rigor and effectiveness of the database search could be increased by narrowing search limits, like the years to the most recent, article type to research articles, and subject areas to nursing and health professions. Also, adding other words to the phrase or keywords from the PICO(T) to refine the search further is essential. The key phrase is electronic health records, and therefore
