Part 1: Professional Disclosure Statement and Informed Consent Welcome! I am so happy you chose me to be your counselor. This document will serve to provide information to you regarding my training background, treatment process, and my billing practices. I am a licensed professional in the state of Texas. I obtained my bachelor’s degree in behavioral health sciences with an emphasis in trauma from Grand Canyon University and my master’s degree in trauma informed care from Texas Christian University. I am trained and have experience in working with trauma, anxiety, depression, marriage and family conflict, parenting issues, shame and guilt, and much more. I utilize a cognitive behavioral approach which focuses on adjusting your way of thinking and maladaptive behaviors by teaching coping skills, problem-solving, and developing healthy self-esteem and building a greater sense of self-worth ( What Is Cognitive Behavioral Therapy? 2017). I am also trained in EMDR therapy. EMDR therapy was created by Dr. Francine Shapiro in 1987 for the treatment of PTSD. It has been proved very effective in the treatment of PTSD and eliminating symptoms in a short period of time – typically anywhere from 6 to 12 sessions. The Therapeutic Relationship: An effective therapeutic relationship is imperative for positive outcomes in therapy. Forming this relationship will take time and require trust to form. Therefore, complete honesty is crucial. You have taken a big step toward healing by seeking out therapy. I want to ensure your time with me is productive because your time is precious, thus feedback regarding the efficacy of the therapeutic relationship will need to be frequent and honest. Lastly, it is important to note that while you and I will be discussing a very personal subject matter, it is important to remember that our relationship is always strictly professional. During our first session, will fill out the PHQ-9 which is a short questionnaire that is a quick assessment on depression symptoms ( Patient Health Questionnaire (PHQ-9 & PHQ-2) , 2020). We will also discuss your symptoms and goals for therapy. Based on your responses to the initial assessment and your symptoms and goals, I will perform another more thorough assessment during a structured interview. For example, if you report a history of trauma, I would administer the CAPS-5 which assesses your symptoms for PTSD. Should you need to get in contact with me outside of sessions, you can call or text me at 972-555-1234. If I am unavailable, please leave me a voicemail and I will get back to you within 24 hours. If you are experiencing an emergency, call 911 or the Collin County Mental Health Authority Crisis Line at 877-422-5939. Possible Benefits and Risks of Treatment: Treatment has innumerable benefits. You will learn coping strategies for tough moments, and you will build skills to help you to adapt to difficult situations. However, therapy does not come without risks. During therapy, you will be discussing upsetting memories for you and experiencing feelings of distress, reliving trauma, anxiety, flashbacks, sleep disturbances, and intrusive memories are all normal reactions and to be expected. While this is far from a comprehensive list, it is important for you to note the possibilities. Please know that I will 2
partner with you as you navigate these difficulties, and I will equip you with the tools you need to be successful. Confidentiality: I am legally and ethically bound to maintain confidentiality with regards to your therapy and anything you disclose to me during sessions. This confidentiality has a few limitations. I must disclose minimal, necessary information regarding you and your treatment to the billing company I work with as well as your insurance company (should you choose to use insurance). Code of Ethics: As a professional counselor, I am bound by the code of ethics set forth by the American Counseling Association. This code can be found at 2014-aca-code-of-ethics.pdf (counseling.org) . Fees: There is a fee of $150 per clinical hour. I accept debit and credit card payments, cash, or insurance. This fee is due at the time of service. I do not accept personal checks. Any insurance co-pay will be due at the time of service. Consent to Treatment: I authorize Stephanie Marcos to provide counseling services. I understand the possible benefits and risks of treatment. I understand that I can ask questions and request a review of my progress at any time. I understand that my participation in therapy is voluntary and that I may discontinue services at any time. I understand that there are no guarantees regarding the success of my treatment. I understand that I am financially responsible for services rendered. Informed consent is when the therapist shares information with the client so that the client can make an informed decision about their treatment and participation in the treatment plan. Informed consent is also a legal document that serves to empower the client by providing them with all information necessary to make informed decisions, reduces risk of exploitation or harm of clients by informing them of appropriate boundaries and guidelines for therapeutic 3
